I tested with asciidoctor-pdf (1.5.4) and worked fine. Which version are you running?

This is my command
 $ asciidoctor -r asciidoctor-pdf -b pdf pdf.adoc


EDIT: I suspect you are replacing the fonts? Maybe the bold font is not setup?
