I've submitted that pull request; it's at https://github.com/asciidoctor/asciidoctor-fopub/pull/84.

I tested the code with a source file named

sample - 3.xml

(This 'fopub.bat' file is the one in the main, 'asciidoctor-fopub' folder.)
