Placement of footnotes in tables

Placement of footnotes in tables

Footnote inside an asciidoc-formatted table cell are rendered in that cell in html output. I think they should be at the end of the table, because footnotes should:

- reduce clutter
- keep the cell size manageable
- avoid repetition of notes that are common to several cells

and the current placement fails at all of these.

The default template should place them either in a final merged cell the same width as the table or in a list directly after it.

As suggested in http://stackoverflow.com/questions/29151711/render-asciidoc-footnote-outside-of-a-table, if this is not to be the default location, could it be an option?
