Generated anchor for headings containing styles.

Take this header as example:

=== This _is_ true!

The generated anchor is:

_this_em_is_em_true

I would have expected:

_this_is_true

An other example:

==== The `HelloWorld` class

Is this decision by design?

This is a known shortcoming that we inherited from AsciiDoc Python.
